Best international health insurance companies in 2026: best for what, with the evidence

IPMIcompare, a free comparison site for international private medical insurance, wrote this guide to say what six international health insurers are each best for, and to show the dated evidence behind every statement instead of an overall number one. The six are Cigna Global, Bupa Global, Allianz Care, AXA Global Healthcare, IMG and APRIL International; the evidence is their own published documents, an independent adviser survey, and IPMIcompare's own premium data from 27 July 2026.

Last updated: 15 October 2026 · Method: read in each insurer's own published documents on 15 October 2026; we rank only where our own price data allows and say so
In short: There is no best international health insurer, only a best fit, and the evidence supports six specific claims: APRIL International has the highest adviser service rating (86.4 of 100, Health & Protection, May 2026); Bupa Global has the highest claims-handling score of the four largest brands (85.8) and the only plan with no annual limit (Ultimate); Cigna Global publishes the most precise premium controls (seven deductibles, four cost-share levels, two out-of-pocket caps); Allianz Care is the one of the six whose moratorium route needs no medical questionnaire; AXA Global Healthcare publishes measured claims speed (83.2% of eligible online claims paid within two days in 2025) and includes evacuation on every level; IMG offers the highest deductibles for catastrophic-only cover (to $25,000) but caps cover per lifetime, not per year. On price, IPMIcompare's own data ranks countries, not insurers: the same person was quoted up to 8.8 times apart on 27 July 2026, so the cheapest insurer is whichever one quotes lowest for you.

The criteria, and what we refuse to rank

A "best" claim is allowed on this page only when one of three sources supports it: the insurer's own published document (plan page, IPID, customer guide, benefit guide, certificate or brochure, read on 15 October 2026, and compared on our feature matrix); the Health & Protection IPMI Service Ratings 2026, an independent survey of almost 200 advisers published in May 2026; or IPMIcompare's own premium data, 17 insurers priced through our rate engines on 27 July 2026 and published as aggregated ranges on the cost page.

We do not rank the insurers overall, because the three sources measure different things and no honest weighting exists. We do not name a cheapest insurer, because we publish ranges, not named premiums, and the cheapest quote changes with age, country and cover. And we do not use third-party rankings or broker commentary: if a claim is not in one of the three sources, it is not here.

Best for what: the table

InsurerBest forEvidence (dated)
APRIL International (MyHealth International)Adviser-rated service: the highest overall score of every provider rated, and the highest claims-handling scoreHealth & Protection IPMI Service Ratings 2026 (May 2026): overall 86.4 of 100, claims handling 87.5, Five Star; next highest overall 80.9.
Bupa GlobalClaims handling among the four largest brands, and cover with no annual limitHealth & Protection 2026: claims handling 85.8, above Cigna Global (77.0), AXA (80.4) and Allianz Partners (76.6). Bupa's Ultimate plan has an unlimited annual maximum (Bupa Global plan pages, 15 October 2026; Hong Kong comparison table from 16 June 2025).
Cigna GlobalDialling the premium preciselySeven inpatient deductibles ($0 to $10,000), four cost-share levels (0 to 30%) and two out-of-pocket maximums ($2,000 or $5,000), plus five outpatient deductibles: the most published premium levers of the six (Cigna Global plans-in-detail and optional-benefits pages, 15 October 2026).
Allianz CareJoining without a medical questionnaireThe only one of the six whose documents state that its moratorium option has "no medical questionnaires to complete"; pre-existing conditions become eligible after a continuous 24-month symptom- and treatment-free period, and full medical underwriting or CPME switch terms are offered instead (Allianz Care plans page and Individual Benefit Guide valid from 1 November 2025).
AXA Global HealthcarePublished claims speed, and evacuation on every levelThe only one of the six that publishes a measured claims figure rather than a target: 83.2% of eligible online claims submitted January to December 2025 were paid within two days (AXA How bills are paid page, 15 October 2026). Evacuation and repatriation are included as standard on all five levels, Foundation included (AXA combined summary of benefits).
IMG (Global Medical Insurance)Catastrophic-only cover with a very high deductible; read the lifetime cap firstDeductibles up to $25,000 on Gold and Platinum, the highest published by the six (next: AXA $16,000, Cigna and Bupa $10,000, APRIL $5,000). The trade-off is that IMG's maximum is per lifetime ($1m Bronze to $8m Platinum), where the other five set annual limits (IMG Global Medical Insurance brochure and Platinum certificate dated 1 January 2026).

Two further facts from the documents that buyers ask about, stated without a "best": the shortest maternity waiting period is 12 months, at Cigna Global (Gold and Platinum; 24 months for some entities), Allianz Care and APRIL, against 18 months at Bupa Global and AXA and 10 or 24 months at IMG; and the longest cooling-off periods are 30 days at Bupa Global and Allianz Care, against 14 days at Cigna, AXA and APRIL and 15 at IMG. Every figure is on the feature matrix with its source.

What our price data can and cannot say

On 27 July 2026 IPMIcompare priced one profile (a healthy single British adult, worldwide excluding the USA, standard tier, inpatient only) through 17 insurers' rate engines in eight countries at four ages, and recorded the lowest, median and highest annual premium. That data can rank markets and show how far apart insurers land; it cannot name a cheapest insurer, because we publish ranges rather than named premiums, and because the ordering changes with age and country.

Source: IPMIcompare rate engines, 27 July 2026, inpatient only, worldwide excluding USA; medians and ranges across 17 insurers. The full table by country and age, with outpatient added, is on what international health insurance actually costs, and the levers that move your own quote are on the cheapest way to find international health insurance.

What the spread means for "best": the widest gap, Singapore at age 60, is 8.8 times between the cheapest and dearest insurer for identical cover. No "best company" list survives that; the best-priced insurer is the one that quotes lowest for your exact profile, which is why this page ends with a quote rather than a winner.

How to choose between them

  1. Start from the claim you are most likely to make. A chronic condition you already have points to the underwriting route (Allianz Care's moratorium or everyone's full disclosure); a planned pregnancy points to the 12-month insurers; a move to Hong Kong or Singapore points to the hospital evidence on our direct-billing page.
  2. Set the limit and the area of cover next. They decide the ceiling; see the matrix row. Excluding the USA is the largest single price lever.
  3. Price the shortlist against the whole market, not against each other. Two insurers that look alike on paper can be quoted thousands of dollars apart; the country drivers explain why.
  4. Check the service score last, not first. It is the only row where the six are measured by the same yardstick, and APRIL's lead is real, but it is advisers' experience, not a claims guarantee.

Frequently asked questions

Which is the best international health insurance company in 2026?
None of them overall. On the evidence IPMIcompare can cite, APRIL International leads on adviser-rated service (86.4 of 100, Health & Protection, May 2026), Bupa Global on claims handling among the four largest brands and on an unlimited plan, Cigna Global on premium controls, Allianz Care on joining without a medical questionnaire, AXA Global Healthcare on published claims speed, and IMG on very high deductibles. The best-priced insurer is whichever quotes lowest for your profile.
Which international health insurer is cheapest?
IPMIcompare's own data cannot name one, by design: we publish aggregated ranges, not named premiums. What the 27 July 2026 data shows is the size of the gap, up to 8.8 times between the cheapest and dearest quote for identical cover in Singapore at age 60, and the cheapest markets by median (Kenya at ages 30, 40 and 60; Nigeria at 50).
Which insurer has the best service rating?
APRIL International, with an overall score of 86.4 of 100 and a claims-handling score of 87.5 in the Health & Protection IPMI Service Ratings 2026, published in May 2026 from a survey of almost 200 advisers. Bupa Global (80.9), AXA Global Healthcare (79.5), Cigna Global (78.5) and IMG (76.9) were also rated Five Star; Allianz Partners scored 71.3, Four Star.
Which insurer is best for pre-existing conditions?
It depends on the condition and on whether you want certainty or speed. Allianz Care is the only one of the six whose moratorium option is stated to need no medical questionnaire, with pre-existing conditions eligible after 24 continuous months without symptoms or treatment; AXA also offers moratorium. Full disclosure, offered by all six, tells you on day one whether a condition is excluded, loaded or covered.
Why does this page not give an overall ranking?
Because the three sources it is allowed to use (the insurers' own documents, an independent adviser survey and IPMIcompare's own premium ranges) measure different things, and no weighting between them would be anything but an opinion. Each "best for" here is one claim with one dated source.
