Hospital networks and direct billing by city: what the insurers and the hospitals publish

IPMIcompare, a free comparison site for international private medical insurance, built this guide to show, for 16 flagship private hospitals in nine cities, which of six international health insurers publish network access or direct billing there, using only the insurer's own provider list and the hospital's own published insurer list. Where neither publishes a fact, the hospital is left out rather than guessed.

Last updated: 15 October 2026 · Method: read in each insurer's own published documents and each hospital's own published list on 15 October 2026 (Cigna's member-portal provider search extracted 5 October 2026); we rank only where our own price data allows and say so (nothing on this page is ranked)
In short: Direct billing is never a property of an insurer alone; it is an agreement between one insurer and one hospital, and the only reliable sources are the insurer's provider directory and the hospital's own insurer list. On 15 October 2026 three of those sources were public enough to tabulate: Cigna Global's member-portal provider search (13,281 hospitals and clinics worldwide, each flagged direct billing yes or no), APRIL International's MyHealth International providers list (March 2026, which names the hospitals where its Explore and Essential plans carry a 40% co-payment), and the hospitals' own insurer pages. The table below gives all three for 16 hospitals in Hong Kong, Singapore, Bangkok, Dubai, London, Paris, Kuala Lumpur, Shanghai and Mexico City. Bupa Global, Allianz Care, AXA Global Healthcare and IMG keep their directories behind a member login, so for them the hospital's list is the only public evidence.

What direct billing means

Direct billing (also called direct settlement or cashless treatment) means the hospital invoices your insurer and you leave without paying the covered part of the bill. It normally needs three things at once: the hospital must have a billing agreement with your insurer, your plan must be one the agreement covers, and the treatment must be pre-authorised (a letter of guarantee). Without any one of them you pay first and claim back. Every insurer in this table says it arranges direct settlement for in-patient treatment where it can; what differs is which hospitals, which plans and whether out-patient care is included.

Two things make hospital lists harder to read than they look. Hospitals list legal entities, not brands: "AXA PPP healthcare Limited" is the underwriter named in AXA Global Healthcare's own UK handbook, "Allianz Worldwide Care" is Allianz Care's former trading name, and "Bupa International" or "Bupa Insurance Services" is Bupa Global. And a listing is not a promise: Mount Elizabeth Hospital in Singapore, for example, says it cannot accept a letter of guarantee from an insurer that has no prior credit arrangement with the hospital, and Bangkok Hospital reserves the right to refuse to work with certain policies or insurers.

How this table was built

Three published sources, one column each, read on 15 October 2026:

  • Cigna Global's member-portal provider search, which lists every hospital and clinic in Cigna's network with a direct-billing flag. IPMIcompare extracted it on 5 October 2026: 13,281 hospitals and clinics worldwide (for the countries in this table: Thailand 81 listings, 74 with direct billing; Singapore 48/42; Hong Kong 73/30; United Kingdom 1,106/869; United Arab Emirates 57/0; Malaysia 88/79; China 213/191; France 209/163; Mexico 72/60). A "No" means the hospital is in Cigna's directory but flagged no direct billing; "not listed" means it was not in the extract.
  • APRIL International's MyHealth International providers list (updated March 2026). It does not mark direct billing per hospital; it names the hospitals where the Explore and Essential plans pay only 60% of costs (a 40% co-payment) and the Extensive and Elite plans pay 100% of reasonable and customary costs, and states that any hospital not listed is covered at 100% on all four plans. Direct billing itself is described at the level of the product: in partner facilities for hospitalisation and planned treatment, and for out-patient care in certain countries.
  • The hospital's own published insurer list: which of the six insurers (Cigna Global, Bupa Global, Allianz Care, AXA Global Healthcare, IMG, APRIL International) it names, in the hospital's wording, with the page and the date it shows. Not named is reported as not named, nothing more.

A hospital appears only if all three cells could be filled from those sources. The six insurers' own policy terms are compared on the feature matrix.

Hospitals and cities left out, and why

  • Bumrungrad International, Bangkok: in Cigna's extract (direct billing No) and APRIL's list (Extensive and Elite only), but its own "We accept" insurer list is served as an image we could not read, so the third cell could not be filled.
  • Bangkok Hospital (headquarters): its international private medical insurance page names no insurers.
  • The London Clinic: its pages say it works with many international providers without naming them.
  • Shanghai United Family Hospital: its direct-billing list is an image.
  • Lisbon (Hospital da Luz) and Cape Town (Mediclinic, Netcare): in Cigna's extract with direct billing Yes, but no hospital-published insurer list was found and APRIL's list has no table for Portugal or South Africa.

If you need one of these, use the three checks below; the facts exist, they are just not published on a page we could cite.

How to check a hospital yourself

  1. Search the insurer's directory for the exact hospital, then read the flag. Cigna's Customer Area search lists providers with direct billing; Bupa's Facilities Finder, Allianz Care's provider finder, AXA's Select network and IMG's Find a Doctor need a login or a quote. A hospital can be in the network without direct billing (Cigna lists 43 Hong Kong hospitals and clinics that way), and a group can have two entries with different flags, as Mount Elizabeth and Gleneagles do above.
  2. Read the hospital's own insurance page and match the entity name. Hospitals name the legal entity on the agreement; see the note on brands above. If your insurer is missing, ask the hospital's insurance desk whether it can bill through an assistance company instead.
  3. Ask for pre-authorisation before admission. Bupa makes it mandatory for all in-patient and day-patient treatment; Cigna reduces what it pays by 20% without it; IMG pays nothing for evacuation and 50% less for other treatment without pre-certification; APRIL applies a 50% penalty to a hospitalisation that was not pre-approved. The letter of guarantee the hospital receives is the direct-billing promise.

Frequently asked questions

Does being in an insurer's network mean the hospital bills the insurer directly?
No. Cigna Global's own provider search flags each listed hospital as direct billing yes or no; in the extract read on 5 October 2026, 43 of its 73 Hong Kong listings and all 57 of its United Arab Emirates listings are flagged no. Network membership tells you treatment is covered at agreed rates; direct billing tells you whether you pay first.
Which hospital in this table names all six insurers?
Gleneagles Hong Kong Hospital: its direct-billing partner list, read on 15 October 2026, shows Allianz Worldwide Care, April International, AXA, Bupa, Cigna and IMG. Cigna's own search nevertheless flags it as no direct billing, which is why both sources are shown.
Why does APRIL's list show a 40% co-payment at some hospitals?
APRIL's MyHealth International providers list (March 2026) names the hospitals where the Explore and Essential plans pay 60% of costs and the Extensive and Elite plans pay 100% of reasonable and customary costs; any hospital not on the list is covered at 100% on all four plans. It is a plan-tier rule, not a direct-billing flag.
Why is Bumrungrad not in the table?
Because its own list of accepted insurers is published as an image we could not read, so the hospital-list cell could not be filled from a source we can cite. The insurer side is known: Cigna's search lists it with direct billing no, and APRIL covers it at 100% on Extensive and Elite only.
Where do Bupa Global, Allianz Care, AXA and IMG publish their hospital lists?
Behind a member login or a quote: Bupa's Facilities Finder, Allianz Care's provider finder, AXA's Select network and IMG's Find a Doctor. That is why, for those four, the only public evidence on this page is the hospital's own insurer list.
